SummaCare Hearing Benefit 2022

Effective January 1, 2022 SummaCare insured members will now have access to a “Formulary Basic” level of hearing aid under their hearing aid benefit in addition to "Formulary Plus." The "Formulary Basic" models will have a $395 per ear member copay. See below for a full listing of hearing aids that fall under each category. In addition, see Levels 2-4, Premiere, and New Releases for options for members to buy-up should they desire.

SummaCare Medicare Advantage HOMELINK Hearing Aids – Member Price

*HOMELINK reserves the right to update hearing aid models with new products released by manufacturers.

Include standard earmolds, one year of participating provider's follow-up services and supplies, and one package of hearing aid batteries. 3-year manufacturer warranty. 3-year loss & damage warranty (one time only with $350 per hearing aid deductible)

  1. Starkey & Widex Rechargeable Hearing Aids require an additional charge – member cost $200 per set
  2. Custom earmold – member cost $75 per earmold

Step 1: Appointment Scheduled

A HOMELINK Patient Care Coordinator will contact you to setup an appointment with a SummaCare member interested in accessing their hearing aid benefit. During this call, detailed instruction will be provided on proper workflows. 

Step 2: Product Selected

SummaCare members have access to a variety of hearing aids subject to a co-pay per hearing aid that will be collected by HOMELINK. See member pricing guide for details. Once the SummaCare member has selected their hearing aid(s), you will need to provide us with the following:  

  • Current hearing test

  • Recommendation form

Summacare members must call HOMELINK to discuss out-of-pocket expenses prior to ordering hearing aid. 

STEP 3: Product Verification and Purchasing 
HOMELINK will contact the SummaCare member to verify their expectations have been met and to set up payment terms. Once completed, HOMELINK will purchase the selected hearing aid(s) and have them shipped directly to your location.   

 

STEP 4: Fitting and Payment 
Once selected hearing aid(s) are fit, return signed HOMELINK SummaCare Medicare Advantage Hearing Aid Agreement and your invoice. Fitting fee includes: 

  • Fitting and 1-year follow up services
  • Standard Earmold(s), if required
  • One package of batteries
  • 60 day trial of hearing aids

 

Please Remember

SummaCare Medicare Advantage hearing aid program is only available through HOMELINK, out-of-network billing is not available. Make sure you call us at 844-358-2549 before providing any services to SummaCare members. 
 

HOMELINK contracts with a variety of payers that require HOMELINK to credential any Facility providing services to their patients. HOMELINK will provide Facility separate electronic notice that a credentialing application must be completed and submitted within fifteen (15) business days of receipt of this notice. Failure of the Facility to submit a complete credentialing application and all supporting documents will result in a loss of referrals and/or payments being held.
